EBRIEF x NTOCC WEBINAR
Using Assistive Technology to Prevent Adverse Outcomes
As patients move between acute and post-acute settings, the risk of "care gaps" increases, often leading to preventable readmissions and functional decline. This session explores how the strategic integration of assistive technologies can stabilize patient health during these transitions as well as, to prevent them from occurring. A primary focus will be placed on the often-overlooked clinical pillar of transition success: high-quality incontinence care. Participants will learn how modern assistive tools and superior incontinence management protocols serve as the "connective tissue" in the continuity of care, preventing falls with major injury and skin damage. Damage related to moisture exposure and the infectious complications that frequently trigger a return to the emergency department or a failed move to a lower level of care.
Objectives:
1. Identify how to utilize sensor-based assistive technologies to detect early warning signs of physiological decline during the first 72 hours post-discharge as it relates to incontinence and its complications to bridge the "information gap" between settings.
2. Analyze the direct correlations between poor incontinence management and common causes of readmission, such as Urinary Tract Infections (UTIs), urosepsis, and Incontinence-Associated Dermatitis (IAD), and falls with major injury.
3. Explore strategies for using digital handoff tools to ensure that specialized care plans are maintained across settings.
